TOPS(R) 1096 2-Part Continuous Form

This 1096 (Annual Summary and Transmittal of U.S. Information Returns) 2-part continuous form is used for transmittal and summary of 1098 1099 and 5498 forms, when the forms are submitted on paper to the IRS. Payers filing returns on paper forms must use a separate transmittal, form 1096 for each different type of form. Ply 1 is for the IRS and Ply 2 is a file copy.
